What is HIPEC Surgery?

HIPEC (Hyperthermic Intraperitoneal Chemotherapy) is an advanced surgical procedure for treating tumors that have spread within the abdominal cavity, especially peritoneal metastases or pseudomyxoma peritonei.
The procedure involves surgical removal of visible tumors followed by circulation of a heated chemotherapy solution within the abdominal cavity. Temperatures reach approximately 41–43°C, enhancing chemotherapy effectiveness while minimizing residual cancer cell spread.

Conditions Treated with HIPEC

HIPEC is typically used for:

    • Peritoneal mesothelioma

    • Colorectal cancer with abdominal metastases

    • Pseudomyxoma peritonei

    • Gastric cancer with peritoneal involvement

    • Selected pancreatic and liver tumors with limited peritoneal spread

This surgery is ideal for patients unsuitable for conventional resection or systemic chemotherapy alone.

Symptoms and Indications Before HIPEC

Patients considered for HIPEC often present with:

    • Abdominal pain and bloating

    • Difficulty eating and significant weight loss

    • Partial or complete bowel obstruction

    • Fluid or gas accumulation in the abdominal cavity

Thorough preoperative evaluation is crucial to select suitable candidates and optimize outcomes.

Diagnostic and Preoperative Evaluation

Dr. Osama Hamed follows a meticulous preoperative assessment, including:

    1. Detailed Clinical Evaluation:
Reviewing medical history, symptoms, and full abdominal examination.

    2. Advanced Imaging:

      • CT or MRI of the abdomen and pelvis to assess tumor size and spread.

      • Occasionally PET-CT to detect metastases elsewhere.

    3. Multidisciplinary Coordination:
Involving oncologists, radiologists, and nutrition specialists to evaluate surgical fitness.

The HIPEC Procedure

The surgery consists of two main phases:

    1. Cytoreductive Surgery:
Removal of all visible tumors in the abdomen, including affected organs if necessary.

    2. Hyperthermic Intraperitoneal Chemotherapy (HIPEC):

      • Heated chemotherapy solution is circulated within the abdominal cavity for 60–120 minutes.

      • Heat enhances drug effectiveness and helps eradicate remaining cancer cells.

⚡ Combining cytoreduction with HIPEC significantly improves disease control and survival compared to chemotherapy alone.
